YOUTH DEVELOPMENT PROGRAM APPLICATION

WAIVER

There is a potential risk for personal injury and damage to property while attending the Youth Development Program.  We at YORK COUNTY BOWMEN endeavor to create and maintain a safe environment at our facility and we have established reasonable control of rules of conduct for participants, spectators and guests that must be followed. Any questions in that regard should be clarified with an official.  This release must be signed by the parents or guardians of a youth participating in the YCB Youth Development Program.

DECLARATION OF MEDIA CONSENT:

PHOTOS, VIDEO & OTHER SOCIAL MEDIA

I, the undersigned, hereby consent to the use of my likeness, biography, picture and activity details related to my person, in photographs made at while engaged in YORK COUNTY BOWMEN activities as well as in publicity concerning the same. 

I consent to the use of my name in connection with the photographs, and I also allow YORK COUNTY BOWMEN not to use my name, as appropriate.

I declare being a major and that I have the full right to make this declaration of consent. In the event that I am a minor, this declaration of consent will be made by the entitled parent or guardian.

I understand that I will not be entitled to receive any payment in consideration for the use of details related to my person as set forth above, in the photographs pursuant to this declaration of consent.

YORK COUNTY BOWMEN shall have the sub-licensable and right to use my details as set forth above in relation with the digital media, in any manner whatsoever. The use may include, but is not limited to, editing, duplication, licensing, distribution and incorporation in other works, in whatever form (e.g. hard copy or electronic), such as posters, publications, web sites, films or videos, and their unrestricted use, without any obligation on the part of YORK COUNTY BOWMEN to seek any further authorization by the undersigned.

RELEASE

I/we have read the above notice of warning and fully understand the risks and conditions of participating in the events and functions organized or sponsored by YORK COUNTY BOWMEN.    I/we hereby save harmless and indemnify YORK COUNTY BOWMEN, its officers, directors, coaches and assistants from
demands, claims, acting suits or proceedings arising out of participation of my/our son/daughter in any club activities or programs. 

I understand and agree that there will be no refunds or credits available for any classes not attended by the participant for any reason.
